John Sheridan fired the Owls in front from the penalty spot before Shane Nicholson equalised with a spectacular effort from distance.
Paul Warhurst regained Wednesday’s lead before Derby scored twice through Marco Gabbiadini and Paul Kitson to put Derby ahead.
But in a game of twists and turns, Wednesday would go on to have the final say and it was Warhurst on target to force a replay at Hillsborough.
The defender-turned-striker proved to be the ultimate match-winner, scoring the only goal in the S6 replay to set up the momentous Steel City semi final at Wembley.
